TG MEDICAL RELEASE FORM
TG cares about your safety and the safety of others. In order to participate in any of our events you must have an updated Medical Release Form on file. These are good for one year and cover you for any/all the TG activities we do in a given year. If you are not sure you have one on file simply contact Rich or download one and fill it out before the next event. It is important to know that if you or your friends show up to an event and do not have a Medical Release Form on file then you will be sent home and unable to participate.
